How to Obtain a Appointment
Step 1:

Step 2:
Once you are signed up for Spruce Health, send us the following message "New Appointment"
A staff member will contact you as soon as possible (during regular business hours).
Step 3:
You will be asked to fill out appropriate new patient questionnaires and paperwork. These MUST be completed BEFORE your first office visit! These will be sent to you via the Spruce App. Once completed, you can either scan and send back or bring them with you to the first appointment.

- Try to arrive to your appointment about 15 minutes early so that our team can review your paperwork and ensure that you are ready to go!
- Bring along an updated and accurate list of medications and/or bring all of your pill bottles along.
- If you have previously seen a neurologist or had any pertinent tests (e.g. MRI, EEG, etc), our staff may ask you to fill out release or records forms so that we can work on having that information available prior to your first appointment.
- Please be advised that your insurance may require a referral from a primary care physician to be seen in our office. You can ask your primary care provider to fax a referral to: 724-203-6472
