If you require your medical records to be sent to another physician, please take the following steps.
PLEASE BE AWARE:
Frequently asked questions and other concerns:
1) What if my doctor does not have a release of records form or simply refuses to mail a form?
You can download a copy of our releases for records form CLICK HERE and mail it to the above PO BOX
2) What if a non-healthcare provider needs the records? E.g. disability attorney, insurance company, workers compensation, etc?
The same principle applies. Those entities will have you sign appropriate release or records forms and these should be mailed to the above PO BOX.
3) I had MRI studies, labs, etc? Where are those studies?
Our office may have copies of your reports but we do not have the physical imaging files to share. You would have to go to your imaging center and ask for the MRI to be burned onto a disc to hand carry to your new neurologist.
4) I never saw Dr. Kozminski but I saw Dr. Elliot Michel at Valley Neurology, what do I do now?
We are custodian of all Valley Neurology records as well and are required to keep records for up to seven years. Thus, if you had seen Dr. Michel, then please have your doctor do a release or records for Valley Neurology and mail to our PO BOX listed above. We will gladly mail any records that we have.

- Ask your physician for a release of records form.
- After signing the form, ask your doctor to mail the completed form to the above PO BOX.
- Once received, we will gladly mail a copy if your records to your physician

- If you had seen Dr. Kozminski while he was employed at Armstrong County Memorial Hospital (before April 2021), you will have to obtain all records from ACMH medical records department.
- If you had seen Dr. Kozminski ONLY as an inpatient consult at ACMH Hospital, likewise, you will have to obtain records from the ACMH medical records department.
